Job Title: Diabetes Education Program Nurse

Flemingdon Health Centre is seeking a highly skilled and compassionate Registered Nurse to join our team as a Diabetes Education Program Nurse. As a key member of our interdisciplinary team, you will play a vital role in delivering comprehensive diabetes management and prevention services to our clients.

Key Responsibilities:
  • Conduct thorough assessments of clients' knowledge, readiness to change, and socio-economic backgrounds to provide personalized education and support.
  • Develop and deliver culturally relevant diabetes education and self-management programs to diverse populations, including South Asians and other equity-deserving groups.
  • Collaborate with the Registered Dietitian and other team members to coordinate and enhance program activities, ensuring equitable access and outcomes for all clients.
  • Participate in community outreach initiatives to promote diabetes prevention and self-management, engaging with local organizations and healthcare providers to expand the program's reach and effectiveness.
  • Utilize program-specific tools to assess client risk levels for diabetes, analyzing data to identify risk factors and inform targeted interventions.
  • Stay current with advancements in diabetes care through continuing education and professional development opportunities, contributing to program reviews and planning, and precepting nursing students and mentoring novice diabetes educators.
Qualifications:
  • Bachelor's degree in Nursing (BScN) and current registration with the College of Nurses of Ontario, in good standing.
  • Certified Diabetes Educator (CDE) certification or a willingness to obtain.
  • Experience in diabetes education, prevention, and management, with knowledge of culturally relevant practices for South Asians and other diverse populations.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a multidisciplinary team.
Working Conditions & Physical Requirements:
  • Incumbent will be expected to work some evenings and/or weekend hours based on project demands.
