Job Summary: We are seeking a highly skilled Registered Dietician to join our team at Mon Sheong Foundation. As a key member of our healthcare team, you will be responsible for providing exceptional nutrition care services to our residents.
Main Responsibilities:- Develop and implement nutrition care plans that meet the unique needs of each resident, ensuring compliance with the Fixing Long-Term Care Act, 2021.
- Collaborate with the healthcare team to identify and address nutrient-related health problems, and develop treatment goals.
- Participate in multidisciplinary care conferences to discuss the nutritional aspects of individual residents and revise the nutritional plan as necessary.
- Assess new admissions and develop resident-centered nutrition care plans within 21 days.
- Ensure that the nutrition and hydration care programs are aligned with the organization's quality standards.
- Eligibility or registration with the College of Dietitians of Ontario.
- Three years of previous professional experience in long-term care.
- Excellent leadership, communication, and interpersonal skills, with a strong interest in gerontology.
- Self-starter with a professional, energetic, and resourceful approach.
- Sensitivity to residents' cultural, spiritual, and physical needs.
