Job Description

Responsibilities
  • Provide patient care and education to individuals and families
  • Collaborate with healthcare professionals to develop and implement care plans
  • Conduct screenings and assessments to identify patient needs
  • Develop and implement individualized care plans to meet patient needs
  • Provide health education and support to patients and families
Requirements
  • Current registration with the College of Nurses of Ontario
  • Experience working in a healthcare setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
