Job Summary

We are seeking a highly skilled Dietitian to join our team at Fraser Health. As a Dietitian, you will play a key role in providing assessment, consultation, and nutrition education to clients and staff in our Obesity Clinic.

Key Responsibilities
  • Conduct patient/client screening, nutritional assessment, and education to Obesity Clinic clients/families.
  • Provide consultation to staff regarding clients with high-risk nutritional needs.
  • Develop a comprehensive shared patient/client plan, in collaboration with the interprofessional team, primary care provider, client, and/or family.
  • Discuss clinical findings with client/family, in collaboration with the interprofessional team, to develop action plans and set goals directed at clinical needs, self-management, self-care, and improved health-related quality of life.
  • Assess learning needs and plan, implement, and evaluate nutrition education programs to address the needs of Obesity Clinic staff and clients.
  • Provide health counseling to clients/families, including education, self-management, self-monitoring, and wellness/health promotion/prevention through a combination of clinic, telephone, or home visits.
  • Maintain clinical records, including intake screens, patient/client assessments, clinical interventions, treatment formulations, care plans, and progress notes.
  • Liaise with nutrition staff from other agencies to ensure that complex nutrition needs of clients are appropriate and can be safely carried out by Home Health and/or Primary & Community Care staff or caregivers in community settings.
  • Collaborate with other members of the team to provide orientation and information to new staff.
  • Participate in research activities regarding nutrition and nutrition-related issues in the community setting as required.
Qualifications
  • Bachelor's Degree in Dietetics or Nutrition from an accredited educational institution, plus successful completion of a community nutrition or dietetics internship program as approved by Dietitians of Canada (DOC).
  • Two years recent related experience in a community setting focused on medical/surgical weight management.
  • Registration with the College of Dietitians of BC.
  • Valid BC Driver's License and access to a vehicle for business-related purposes.
