About the Role:
We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker, Inpatient Psychiatry, you will play a vital role in providing psychosocial assessment and therapeutic clinical counseling services to clients in psychiatry.
Key Responsibilities:
- Conduct psychosocial assessments of clients through focused interviews and interpretation of data within the context of the client system.
- Provide supportive counseling services to clients, identifying areas of client/system strengths and challenges, and offering interventions which promote improved psychosocial/emotional wellbeing.
- Develop and implement plans of care, including goals of treatment, and make necessary referrals for further intervention.
- Educate and provide consultation to clients and healthcare professionals on issues related to Social Work practice in the psychiatric setting.
- Maintain client records, documenting client assessments, preparing progress notes, and writing reports in accordance with established professional/organizational standards.
- Collaborate with the interdisciplinary team in client care planning and identify clients who would benefit from Social Work intervention.
- Act as a liaison between psychiatric unit and community agencies, facilitating care conferences and providing education regarding community resources.
Qualifications:
- Bachelor's Degree of Social Work from an accredited school of Social Work.
- One year's recent, related experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's license and access to personal vehicle for business related purposes.
