We are seeking a highly skilled and experienced Clinical Transfer Nurse to join our Patient Transfer and Flow Office team at Northern Health Authority. The successful candidate will be responsible for coordinating patient transfers between health authority facilities, ensuring timely and effective delivery of transportation resources to support patient flow through the healthcare continuum.
Key Responsibilities- Coordinate patient transfers between health authority facilities, utilizing transportation systems to facilitate safe, timely, and cost-effective transfers and repatriations.
- Apply the nursing process of assessment, planning, implementing, evaluating, and documenting program services to all patients requiring transfer.
- Advocate for patients, negotiate with regional access representatives, and ensure adherence to provincial policy and escalation where necessary.
- Collaborate with Patient Transfer Leads, Cardiac Triage Coordinator, hospital staff, physicians, bed utilization coordinators, other health authorities, and transportation agencies to ensure coordination and timely delivery of transportation resources.
- Escalate transfer-and flow-related issues to achieve logical solutions and mitigation strategies.
- Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Minimum two (2) years' recent acute care experience, including coordination of inter-facility patient transfers.
- Related transfer and/or bed management experience and emergency and/or critical care experience preferred.
- Computer skills with knowledge and expertise in Microsoft Office and medical electronic record applications/systems and other relevant software and/or database applications.
- Excellent interpersonal skills and the ability to maintain effective working relationships with others internal and external to the organization.
- Demonstrated ability to communicate effectively with patients, family, the public, clinical staff, and members of the inter-disciplinary team.
- Fosters an environment that values patient and family-centered care.
- Demonstrated ability to work independently and as a member of a team.
- Ability to work effectively under pressure with changing priorities and deadlines; ability to resolve conflict.
- Excellent oral and written communication skills.
