Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans for patients.
- Diagnose and manage the care of patients with complex health needs.
- Develop and implement strategies to promote health and prevent illness in individuals, families, and groups.
- Deliver educational programs and workshops to promote healthy lifestyles and disease prevention.
- Collaborate with healthcare teams to provide comprehensive care and support.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Master's degree in Nursing or related field.
- Registered Nurse (EC) certification and Nurse Practitioner (NP) Adult or Primary Care certificate.
- Current Certificate of Competence with College of Nurses of Ontario.
- Over 10 years of experience in primary and community care and health promotion.
- Experience in nursing research and quality improvement methodology.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
