Job Summary

We are seeking a skilled Community Health Nurse Specialist to join our team at Island Health. As a key member of our community outreach team, you will provide intensive case management services to adults with mental health disorders.

Key Responsibilities
  • Assess and develop treatment plans for clients with complex mental health needs;
  • Provide case management services to support clients in achieving their goals and improving their overall well-being;
  • Collaborate with community partners and healthcare providers to ensure comprehensive care;
  • Develop and maintain relationships with clients, families, and community stakeholders;
  • Stay up-to-date with relevant legislation, policies, and best practices in mental health and addictions.
Requirements
  • Graduation from a recognized nursing program and a minimum of one year's recent psychiatric nursing experience;
  • Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant;
  • Current CPR certification;
  • Knowledge of Psychosocial Rehabilitation (PSR) model, assessment and treatment methodologies, and case management principles;
  • Ability to communicate effectively, work independently, and collaborate with others;
  • Physical ability to perform the duties of the job.
