Patient Care Coordinator Mental Health Specialist

Fraser Health is seeking a highly skilled Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for providing clinical supervision, guidance, and support to nursing staff, other clinical team members, and support staff within the assigned unit(s). Your expertise will be instrumental in ensuring the delivery of high-quality patient care and promoting a culture of excellence in practice.
Key Responsibilities- Apply established criteria and prioritize admissions/intake, lead and facilitate actions related to daily patient flow, plan for patient discharge, and ensure the care team follows through on specific care paths and processes.
- Supervise nursing and nursing support staff, coordinate the work of the clinical team, and monitor the quality of care and the safety of patients.
- Support quality patient care and excellence in practice by promoting interprofessional collaboration, team building, and staff education.
- Initiate/lead/support plan of care conferences and rounds for optimal patient care management.
- Identify learning requirements of staff and provide input into the development of learning objectives.
- Participate in quality improvement and risk management activities, and collaborate with internal and external stakeholders.
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, or an equivalent combination of education, training, and experience.
May require current Class 5 Driver's License and access to personal vehicle for business-related purposes for applicable programs.
