About the Role

We are seeking a skilled Pharmacist - Patient Care Specialist to join our pharmacy team. The ideal candidate will be passionate about taking care of the health and well-being of individuals and families in our communities.

In this role, you will be responsible for demonstrating warmth, empathy, and compassion while acting in the patient's best interest to achieve excellence in patient-centered care, customer service, workflow efficiencies, and professional clinical services.

You will provide patient counselling and education for new therapy to patients/caregivers while tailoring to their needs and concerns. Additionally, you will initiate care plans, follow up, and complete proper documentation.

The successful candidate will participate in professional programs and collaborate with colleagues to provide excellence in health & wellness services to all patients. They will also ensure prescription safety prioritizing technical functions to Pharmacy Registered Technicians and Assistants as required.

This role requires a thorough understanding of relevant corporate pharmacy service offerings/programs and the ability to provide each customer/patient with the program that meets their healthcare needs.

Requirements
  • Bachelor of Science in Pharmacy from a recognized institution
  • Licensed Pharmacist, in good standing, in the province of responsibility
  • Injection and First Aid Certification
  • Kroll software experience is preferred
