About the Role:

The Alberta Health Services is seeking a skilled Social Worker II to join our team in the Seniors Mental Health Program. As a member of our inter-disciplinary team, you will provide a range of social work services to patients and their families, including psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Key Responsibilities:

  • Provide psychosocial assessments and develop treatment plans to address the needs of patients and their families.
  • Conduct individual, group and family therapy sessions to promote mental health and well-being.
  • Collaborate with the multidisciplinary team to provide comprehensive care and support to patients and their families.
  • Develop and implement resource-based interventions to address the social determinants of health.
  • Provide risk screening and assessment to identify patients at risk of harm to themselves or others.
  • Develop and implement case management plans to support patients and their families in achieving their goals.
  • Provide transition or discharge planning to ensure a smooth transition for patients and their families.
  • Develop and implement prevention, promotion and public awareness activities to promote mental health and well-being in the community.

Requirements:

  • Completion of a bachelor's degree in Social Work.
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Preferred Qualifications:

  • Experience working in a mental health setting.
  • Knowledge of the Alberta Mental Health Act and related legislation.
  • Experience working with seniors and their families.
