Job SummaryWe are seeking an experienced Healthcare Professional to join our Home Health team as a Care Coordination Specialist. In this role, you will work closely with clients, healthcare teams, and community agencies to provide comprehensive care coordination services.
About the RoleThis position involves working with a select population of eligible clients who are chronically ill or have long-term physical disabilities. You will use your expertise in chronic disease management and self-care principles to conduct assessments, identify problems, address issues or variances from the plan of care in collaboration with the interdisciplinary team and/or community agencies.
- You will manage client care plans, ensuring continuity across the continuum by liaising with the interdisciplinary team, coordinating clinical interventions, evaluating client outcomes based on client goals for care, and coordinating ongoing care and support.
- You will develop and maintain formal partnerships with General Practitioners as appropriate and in accordance with CHA service delivery models.
- You will act as a clinical resource for the interdisciplinary team, providing information on nursing theory and practice related to clients with complex healthcare needs.
Home Health programs provide a range of healthcare and support services to people living in the community who have acute, chronic, palliative, or rehabilitative healthcare needs. Our clients typically have ongoing health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources.
RequirementsTo be successful in this role, you must have:
- A current practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent clinical nursing experience working in acute care with a medical and/or geriatric focus, recent case management or home care nursing experience or an equivalent combination of education, training, and experience.
- Local area travel requires the use of a personal vehicle.
- A valid BC Driver's License.

Estimated Salary RangeThe estimated salary for this position is CAD $53,611 - CAD $65,513 per annum, depending on experience.
