Job Summary

We are seeking an experienced Community Health Nurse Leader to join our team in Oakville. As a key member of our leadership team, you will be responsible for providing clinical direction, guidance, and support to our nursing staff, while also contributing to the development and implementation of our care programs.

Key Responsibilities
  • Provide hands-on clinical care to clients in their homes, including wound care, medication management, and symptom control
  • Supervise and mentor a team of nurses, providing coaching and feedback to ensure excellent patient outcomes
  • Develop and implement care plans that meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure seamless transitions of care
  • Conduct regular audits and quality improvement activities to ensure compliance with regulatory requirements
Requirements
  • RPN registration in good standing
  • Current CPR certification
  • 10+ years of experience as a nurse, preferably in a community or leadership role
  • Excellent communication and interpersonal skills
  • Ability to work independently and lead a team
  • Strong analytical and problem-solving skills
