Job Description

We are seeking an experienced Nurse Practitioner Lead to join our GAIN Community Team in a full-time capacity. The successful candidate will be responsible for coordinating and providing healthcare services to older adults and frail seniors, working collaboratively with physicians, nurses, and other healthcare professionals.


Responsibilities
  • Clinical Care and Consultation Services: Provide comprehensive geriatric assessment using the GAIN model of assessment tools, autonomously diagnose, order and interpret diagnostic tests, prescribe pharmacological agents, and perform specific procedures appropriate for geriatric clients within the legislated scope of practice.
  • Client Care Planning: Plan, promote, and coordinate evidenced-based care for older adults and frail seniors with complex needs, collaborating with the inter-professional team, primary care providers, Geriatricians, SPLC's community support services, and external providers (Home and Community Care and community Palliative Care Teams).

Required Skills and Qualifications
  • A Degree in Nursing, and successful completion of the Ontario Primary Healthcare Nurse Practitioner Certificate Program.
  • Membership in good standing with a regulatory body in Ontario (College of Nurses of Ontario, Nurse Practitioner Association of Ontario).
  • Canadian certificate in Gerontology an asset.
  • Current CPR and First Aid Certificates.
  • Training in PIECES TM, Gentle Persuasive Approach, U-First, Montessori, an asset.
  • Training/knowledge of GPA, P.I.E.C.E.S. TM, U-First, Recovery/Psychosocial rehabilitation.
