Key Responsibilities- Conduct initial and ongoing assessments to identify client needs and develop effective care plans
- Provide direct psychosocial care, counseling, and group work to promote client well-being
- Collaborate with healthcare providers and community resources to ensure seamless care coordination
- Develop and maintain relationships with clients, families, and community partners to promote person-centered care
- Participate in quality improvement initiatives to enhance patient outcomes and experience
- Bachelor's degree in Social Work from an accredited university
- Two years of recent experience in healthcare or a related field
- Current registration with the British Columbia College of Social Workers
- Valid BC driver's license
