### Job Summary

We are seeking an experienced Social Worker to join our team at the Penticton Regional Hospital. As a Social Worker, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. You will work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, and group work, system navigation, and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.

### Key Responsibilities

* Provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families
* Work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, and group work
* Provide system navigation and advocacy, education, and linkage to relevant specialized complex care resources
* Facilitate client self-management
