Nurse Care Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team at ComForCare Home Health Care - Halton. As a Nurse Care Specialist, you will play a vital role in providing high-quality patient care and support to our clients.
Key Responsibilities:- Comprehensive Assessments: Perform thorough assessments of client status, including physical, psychosocial, and environmental parameters.
- Individualized Care Plans: Develop and implement individualized care plans that incorporate analysis of data and current scientific findings.
- Interventions and Reporting: Provide interventions with a focus on achieving realistic client outcomes and report changes to update the care plan.
- Medication Administration: Administer medications as needed and maintain accurate client records.
- Referrals and Collaboration: Initiate referrals to other health team members as needed and maintain effective communication.
- Education and Licensure: Graduate of an accredited school of professional nursing and current license as a Registered Nurse.
- CPR Certification: Current CPR certification and access to reliable transportation.
- Confidentiality and Safety: Maintain confidentiality in all aspects of the job and adhere to safety and infection control practices.
