Job Summary:

We are seeking compassionate and dedicated Personal Support Workers (PSW) Health Care Aides (HCA) or Continuing Care Assistants (CCA) to join our award-winning team.

In this role, you will provide direct personal support and care to patients, addressing their physical, intellectual, emotional, social, and spiritual needs in accordance with our patient-centered policies and procedures.

Key Responsibilities:

  • Provide personalized care to patients, including assistance with ambulation, feeding, bathing, and grooming.
  • Implement and follow care plans as directed by nursing staff.
  • Document patient care activities and observations accurately and promptly.
  • Communicate effectively with patients, families, and other healthcare team members.
  • Create a safe, comfortable, and dignified environment for patients.
  • Participate in ongoing training and professional development opportunities.

Qualifications and Requirements:

  • Completion of a PSW Program or HCA/CCA certification that meets appropriate standards.
  • Experience in Long Term Care or community home care setting (preferred).
  • Knowledge of dementia and palliative care (preferred).
  • Familiarity with care planning systems and documentation.
  • Strong communication skills and ability to interact positively with staff, residents/patients, and families.
  • Proficiency in reading, writing, and understanding verbal and written instructions in English.
  • Ability to work independently, especially in a Community Home Care setting.
  • Access to a reliable vehicle for community home care (required).
