Clinical Social Worker

Job Summary
This role is responsible for providing clinical social work services to patients, families, and healthcare teams in a dynamic and interdisciplinary setting. The successful candidate will work collaboratively with healthcare professionals to provide comprehensive assessment, treatment planning, and supportive counseling to patients and families.
Key Responsibilities
- Provide comprehensive assessment and treatment planning for patients and families
- Offer educational and supportive counseling to patients and families
- Identify and link patients and families with community resources and services
- Collaborate and consult with healthcare teams and community-based service providers
- Participate in education and professional development opportunities to enhance skills and knowledge
Requirements
- Member of the Ontario College of Social Workers & Social Service Workers or candidate for certification
- Master of Social Work from an accredited School of Social Work or equivalent degree
- Recent related experience in Maternal Newborn and Pediatric environments an asset
- Knowledge of CAS and community resources
- Ability to develop and maintain positive working relationships with healthcare teams
