About the Role

We are seeking a dedicated Home Health Care Provider to join our team at CarePartners.

As a Home Health Care Provider, you will play a vital role in providing patient-centered care, promoting independence, and enhancing the overall well-being of our clients.

Key Responsibilities:
  • Assist with daily living activities, such as personal care, household tasks, respite care, and medication assistance.
  • Promote a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety practices.
  • Provide cognitive, social, and emotional support to patients.
Requirements:
  • A strong desire to make a difference in the lives of others.
  • Exceptional interpersonal skills and ability to work independently and as part of a team.
  • Availability to work non-traditional hours, including early mornings, evenings, and alternate weekends.
  • A valid driver's license and vehicle.
  • A valid certificate as a Personal Support Worker or equivalent qualification.
