About the Role: As a Registered Nurse in our Home Hemodialysis Program, you will play a vital role in shaping the future of healthcare. With a strong focus on patient-centered care, you will work collaboratively with our multidisciplinary team to provide comprehensive support to patients with chronic kidney disease, empowering them to achieve their goals and live independently at home.

Key Responsibilities:

  • Perform patient assessments, education, and training to ensure patients have the necessary knowledge and skills to perform dialysis independently.
  • Coordinate clinical activities, act as a clinical resource/role model, and work directly with patients and the team to ensure timely and appropriate treatment and follow-up.
  • Participate in a rotational on-call support for the program, including coverage in the Long Term Care dialysis units.
  • Liaise closely with the multidisciplinary team to ensure seamless care coordination.

Requirements: To be successful in this role, you will need:

  • Current registration with the College of Nurses of Ontario.
  • BLS Provider certification required.
  • CNeph Certification preferred.
  • Minimum of 2 years recent nephrology experience, with at least 18 months in hemodialysis.
  • Previous experience in pre-dialysis, transplantation, or home hemodialysis is an asset.
