Job Title: Registered Practical Nurse - Complex Continuing Care

We are seeking a compassionate and skilled Registered Practical Nurse to join our team at The Salvation Army - Toronto Grace Health Centre. As a member of our Continuing Complex Care Unit, you will provide high-quality patient care to individuals with complex medical needs.

Job Summary:

The Registered Practical Nurse provides excellent patient and family-centered nursing care in a complex patient environment. Establishes caring, therapeutic relationships with patients and their families to attain or maintain optimal functioning.

Responsibilities:
  • Provide nursing care to patients with complex medical needs, including ventilated patients and those with tracheostomies.
  • Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
  • Establish and maintain therapeutic relationships with patients and their families.
  • Participate in ongoing education and training to enhance skills and knowledge in complex care.
Requirements:
  • Current Certificate of Competence issued by the College of Nurses' of Ontario in good standing.
  • Current Basic Cardiac Life Support (BCLS) Certification.
  • Minimum 1 year of nursing experience, with experience in continuing complex care.
  • Experience working with medically complex patients in acute care hospitals.
  • Ability to work as a member of the chronic care team within the Unit's mission statement.
