Job Title: Community Health Nurse

At WRHA Community Health Services, we are seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will provide high-quality nursing care to clients in the community, including those with complex and unpredictable health needs.

Responsibilities:
  • Provide independent nursing services to clients in the community, including administration of prescribed treatments and leadership roles within the healthcare team.
  • Facilitate coordination of client care and discharge, ensuring seamless transitions between healthcare settings.
  • Work collaboratively with other healthcare professionals to provide comprehensive care to clients.
  • Develop and implement individualized care plans to meet the unique needs of clients.
  • Monitor and evaluate client outcomes, making adjustments to care plans as needed.
Requirements:
  • Graduate of an approved Registered Nursing education program.
  • Current registration with the College of Registered Nurses of Manitoba (CRNM).
  • 2 years of recent experience in community health nursing, preferably with a focus on complex and unpredictable health needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to prioritize care for multiple clients, delegate tasks to support staff, and manage competing demands in a fast-paced environment.
  • Valid Manitoba Class 5 driver's license and reliable vehicle.
Preferred Qualifications:
  • CNA Community Health Nurses Certification or relevant applicable certification.
  • Additional wound care and chronic disease management education.
  • Knowledge of Indigenous customs, traditions, and values, with the ability to communicate effectively with Indigenous clients and families.
