About the Role:

VHA Home HealthCare (VHA) is a leading provider of home care services in Ontario. Our team is dedicated to delivering high-quality, client-centered care to those who need it most.

As a Nursing Professional - Child & Family Program Specialist, you will provide personal care and medical interventions to children with complex medical needs in their homes or in school settings. You will work collaboratively with our interdisciplinary team to develop and implement care plans that meet the unique needs of each client.

Key Responsibilities:

  • Assess, monitor, and make care planning and coordination
  • Evaluate care plan interventions through ongoing reassessment and evaluation results
  • Ensure the quality of care of patients by identifying and acting on factors that affect practice autonomy and delivery of quality care
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families
  • Support client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement

Requirements:

  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Membership of your Professional Association preferred (RNAO/WeRPN)
  • RN diploma/degree or RPN diploma from a recognized university or college
  • A valid First Aid or CPR Certification
  • A moderate level of wound care or venipuncture knowledge and experience is an asset
  • Acute Care and Medical-Surgical nursing experience is an asset
  • Knowledge of community nursing is an asset
