About the Role

CarePartners is seeking a highly skilled Occupational Therapist to join our dynamic Rehabilitation Team. As an Occupational Therapist, you will play a critical role in providing high-quality patient care and supporting the growth of our therapy/rehabilitation program.

Key Responsibilities
  • Provide professional assessment of patients in the community and in their homes
  • Develop and maintain effective Care Plans with respect to Community Health Services
  • Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team
  • Provide one-on-one therapeutic support with clients and promote independence in the home and community
  • Collaborate with the Community Support Workers in the field and provide coaching and supervision as needed
