Advanced Mental Health Outreach Professional in Vancouver

We are seeking a skilled Mental Health and Addictions Outreach Nurse to join our Assertive Community Treatment (ACT) team at Vancouver Coastal Health. This is an exciting opportunity to work as part of a multidisciplinary team, providing triage and outreach services that are inclusive and flexible for individuals with mental illness, addictions, and/or chronic medical conditions.
In this role, you will conduct mental and physical status assessments, assess clients' physiological, psychological, sociocultural, and spiritual needs, and determine priority of care requirements based on client needs, availability of resources, and best practices. You will also provide counseling, assist clients with reducing repeated contact with acute and community healthcare services/programs/resources, assess high-risk situations, and provide crisis intervention.
As a member of the interdisciplinary team, you will develop, implement, and adjust client care plans and meet the client's need for integrated services and continuity of care across the continuum. You will facilitate the movement of clients between acute care, community agency support sites, and designated health centers. Additionally, you will participate in case conferences/meetings with members of the multidisciplinary team and assist clients with attending needed services such as medical appointments.
The successful candidate must have a current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM), a diploma in Nursing or Psychiatric Nursing, and two years' recent related experience working with individuals with mental health and addictions illness or an equivalent combination of education, training, and experience.
