We are seeking a highly skilled and compassionate Outreach Support Worker to join our team at Fraser Health. As a key member of our multidisciplinary team, you will provide community-based support services to individuals experiencing mental health, behavioral, and developmental challenges.
Key Responsibilities- Provide outreach support services, including life skills training, coaching, and advocacy to clients and their caregivers/families.
- Assist in mental health and substance use treatment, and provide support services to promote independence and self-sufficiency.
- Observe, interview, and assess client behavior to identify problems and needs, and respond accordingly.
- Develop and implement short-term strategies to address various situations, and provide feedback and recommendations to treatment plans.
- Document client progress and maintain accurate records.
- Participate in multidisciplinary team meetings and contribute to quality improvement initiatives.
- Grade twelve plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma.
- Two years' recent related experience, or an equivalent combination of education, training, and experience.
- Current CPR certificate and valid Class 5 BC Driver's Licence.
- Knowledge of legislation such as the Mental Health Act and the Guardianship Act.
- Ability to communicate effectively, both verbally and in writing.
- Ability to deal with others effectively, and prioritize and organize client demands and care needs.
- Ability to problem-solve, teach life skills, and demonstrate sound judgment and good observation skills.
