Physiotherapist Job Description

CarePartners is a leading provider of home health care services in Ontario, offering a range of services including nursing, personal support, therapy, and rehabilitation support. We are currently seeking a highly skilled Physiotherapist to join our dynamic Rehabilitation Team.

Key Responsibilities:
  • Conduct professional assessments of patients in the community and in their homes
  • Develop and maintain effective care plans in collaboration with the Home and Community Care Support Services (HCCSS)
  • Work with patients to set and achieve time-specific, measurable goals and outcomes
  • Provide one-on-one therapeutic support to clients
  • Promote independence of clients in the home and community
Requirements:
  • Master's Degree in Physiotherapy from a recognized university
  • Registration with the College of Physiotherapists of Ontario
  • Proven computer skills, including email, word processing, and work scheduling systems
  • Strong assessment skills and ability to participate in effective treatment programs
  • Excellent English verbal and written communication skills
  • French language skills are an asset
