About the Role

We are seeking a skilled Speech Therapist to join our team as a Community Care Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of individuals and families across North Wellington County.

Responsibilities
  • Assess and implement speech language interventions in a home-based setting, utilizing a collaborative and interdisciplinary team-based approach to care.
  • Support a holistic and restorative approach to care, incorporating enhanced stroke pathways and cognitive communication supports.
  • Leverage digital health platforms to support care delivery, collaboration, documentation, and intervention.
Requirements
  • Registration as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario.
  • Valid driver's license and vehicle insurance.
  • Prior experience delivering care both in-person and using virtual methods/technology.
  • Excellent communication skills and ability to work collaboratively within an interdisciplinary team.
  • Flexibility and critical thinking skills to apply to various situations.
