Job Title: Social Worker - Family Support Specialist

Job Summary:

The Darling Home for Kids is seeking an experienced Social Worker to provide direct social work services to families of children with medical complexities. This role involves providing support, education, and advocacy to families, as well as collaborating with healthcare teams to ensure comprehensive care.

Key Responsibilities:

  • Provide direct social work services to children, adolescents, and families of children who are medically fragile and technology-dependent.
  • Advocate, educate, and provide consultation to meet the complex needs of clients, families, and healthcare teams.
  • Develop, implement, and oversee comprehensive social work treatment plans and interventions.
  • Assess psychosocial needs, develop plans of care, and provide client/family education and support groups.
  • Provide service navigation support, maintain knowledge of available resources, and disseminate information to families.
  • Oversee projects relating to the support and education of clients and families, such as hosting education sessions and developing resources.

Qualifications:

  • Membership in OASW and registration with OCSWSSW required.
  • M.S.W. experienced in working with children, adolescents, and their families, as well as individual, couple, and family counseling.
  • Ideal candidate must have an undergraduate degree in business, life sciences, health informatics, or a related discipline.
  • Project Management Professional (PMP) designation considered an asset.
  • Clinical pediatric experience in inpatient or outpatient settings preferred.
  • Knowledge of all appropriate legislation essential.
