Job Description

As a Registered Nurse - Hospital to Home, you will be responsible for providing high-quality patient care in a community setting. This role requires a strong understanding of nursing principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Provide general and specialized nursing duties to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans as needed
Requirements
  • Bachelor of Science in Nursing (BScN) or equivalent
  • Current registration with the College of Nurses of Ontario
  • Minimum two years of acute hospital experience as a Registered Nurse
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Proficiency in electronic medical records and other healthcare software
Preferred Qualifications
  • Experience working with diverse patient populations, including multicultural, homeless, palliative, and geriatric patients
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
