As a Registered Nurse - Hospital to Home, you will be responsible for providing high-quality patient care in a community setting. This role requires a strong understanding of nursing principles and practices, as well as excellent communication and interpersonal skills.
Key Responsibilities- Provide general and specialized nursing duties to patients in their homes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient progress and adjust care plans as needed
- Bachelor of Science in Nursing (BScN) or equivalent
- Current registration with the College of Nurses of Ontario
- Minimum two years of acute hospital experience as a Registered Nurse
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Proficiency in electronic medical records and other healthcare software
- Experience working with diverse patient populations, including multicultural, homeless, palliative, and geriatric patients
- Case management experience or recent related community experience
- Ability to speak French or another second language
