Social Services Coordinator

Fraser Health Authority is seeking a dedicated and compassionate Social Worker to join our team. As a Social Services Coordinator, you will work closely with patients, families, and healthcare professionals to provide comprehensive social work services and therapeutic clinical counseling.
About the RoleThe ideal candidate will have a Master's degree in Social Work from an approved school and at least one year of recent, related acute care clinical experience. You will be responsible for conducting comprehensive social work and psychosocial assessments, providing therapeutic services, and collaborating with the interdisciplinary team to develop and coordinate care plans.
Responsibilities- Conduct comprehensive social work and psychosocial assessments of patients and families
- Provide therapeutic clinical services, including grief and bereavement counseling
- Consult and educate patients, families, and healthcare professionals on issues related to high risk factors, recovery, and current practice in the field of adaptation
- Maintain patient records and document information in accordance with established standards, policies, and procedures
- Master's degree in Social Work from an approved school
- At least one year of recent, related acute care clinical experience
- Current full registration with the BC College of Social Workers
- Comprehensive knowledge and understanding of emotional and mental health issues related to illness, injury, and patient and family cultural dynamics
- Ability to educate and provide consultation to patients, families, and healthcare professionals
