About the Role

This exciting opportunity is for a Care Coordinator to join our multidisciplinary team. As a Care Coordinator, you will play a vital role in assessing patient care needs and determining eligibility for services. You will provide access and referrals to community resources, engage with patients, caregivers, and healthcare practitioners, and coordinate care plan delivery.

You will work collaboratively with patients and their families to develop individual care plans, link patients with service providers, establish helping relationships, and balance patient needs with available resources. Your excellent interpersonal, communication, assessment, problem-solving, and decision-making skills will be essential in this role.

We welcome applicants with experience in community health or a related field who possess knowledge of the healthcare delivery system and community resources. Proficiency in both English and French is essential, and we offer competitive compensation packages and benefits.
