Job Summary:

This full-time position as a Care Coordinator requires an experienced registered nurse, physiotherapist, occupational therapist, or social worker who can assess and determine patient care needs and eligibility. You will provide access and referrals to community services, engage with patients, caregivers, and other healthcare practitioners.

As a valued team member, your primary responsibility will be to help our patients receive healthier care at home while benefiting from our collaborative team's support for professional growth, personal wellness, and work-life balance.

Key Responsibilities:

  • In collaboration with patients and their families, you will assess care needs, determine eligibility for services, and develop individual care and service plans.
  • You will link patients with service providers and coordinate the delivery of care plans.
  • Your role will involve establishing a helping relationship with patients and their families and balancing patient needs and choices with available resources.
  • You will also ensure that patients' values and preferences are respected.

Requirements:

  • You must hold membership in good standing with the applicable regulatory body, such as the College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, or Ontario College of Social Workers and Social Service Workers.
  • You should have recent experience in community health or a related field.
  • You should possess excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • You must be able to effectively manage time, prioritize tasks, and maintain accurate documentation and reports.
  • A valid driver's license and reliable vehicle are necessary for this position.
  • Advance oral and written proficiency in English is essential.
