Position Summary


The Fraser Health Authority is seeking a skilled Licensed Practical Nurse to join our team in a full scope position.


Key Responsibilities



  • Conduct comprehensive assessments of patients to identify their actual and potential strengths and limitations.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Provide nursing care according to identified policies and procedures, technology, and FHA standards, and evaluate outcomes.
  • Participate in multidisciplinary care planning meetings and family conferences to ensure patient-centered care.
  • Document and update all relevant care and procedure information to ensure patient safety and comfort.
  • Maintain a safe and clean environment for patients, staff, and visitors.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Participate in continuous quality improvement and program evaluation activities.
  • Perform other related duties as assigned.

Qualifications



  • Graduate of a recognized practical nursing program or equivalent combination of education, training, and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to deal with others effectively.
  • Ability to organize work and implement a plan of care.
  • Knowledge and competencies to work at full scope of practice as currently required by the BCCNM.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
