Bloom Care Solutions is seeking a highly skilled and experienced Clinical Care Experience Supervisor to join our team. As a key member of our leadership team, you will be responsible for overseeing the delivery of high-quality care services to our clients.

Key Responsibilities
  • Leadership and Supervision: Lead and direct a team of Personal Support Workers to deliver exceptional client-focused service that meets the highest standards of practice and company policies.
  • Care Planning and Coordination: Develop and ensure that care plans are available for clients as required, and visit clients to update care plans as necessary.
  • Training and Development: Conduct training sessions and facilitate educational opportunities for Personal Support Workers to enhance their skills and knowledge.
  • Client Communication and Concerns: Ensure that all client concerns and complaints are addressed and documented in a timely manner.
  • Regulatory Compliance: Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body and ensure compliance with all relevant regulations.
  • Collaboration and Communication: Work closely with Coordinators responsible for scheduling Personal Support Workers and maintain open communication with all team members.
  • Recruitment and Performance Management: Work with recruitment to select, orient, and train new Personal Support Workers, and participate in performance management and discipline as required.
  • Confidentiality and Health and Safety: Maintain confidentiality of client, PSW, and corporate information and participate in proactive Health & Safety activities.
Requirements
  • A regulated health professional RN/RPN with a minimum 2 years experience.
  • Preferably 2-3 years recent experience with progressive responsibilities as a manager in the community setting.
  • Solid knowledge of principles, practices, and methods of community nursing and service delivery to clients.
  • Able to supervise a team of Personal Support Workers.
  • Valid drivers license and evidence of current CPR certification.
  • Strong time management skills, confidentiality, and professionalism.
  • Excellent communicator, able to foster relationships with Home Care Staff and clients.
  • Ability to work independently and work effectively within a team environment.
  • Interest in continuous learning – high energy and focused.
  • Police Criminal Record Check with Vulnerable Sector Screening.
