Job Title: Registered Dietitian

Southbridge Care Homes is seeking a highly skilled and compassionate Registered Dietitian to join our team.

Job Summary:

The Registered Dietitian will be responsible for providing nutrition assessment, care planning, and implementation for residents in our long-term care facilities. The ideal candidate will have a strong background in geriatric nutrition and experience working in a long-term care setting.

Responsibilities:
  • Conduct nutritional assessments for residents using RAI MDS 2.0
  • Determine individualized resident diets and nutritional interventions
  • Develop, implement, and evaluate nutritional care plans
  • Maintain excellent communication with the Dietary Manager
  • Review seasonal menus for nutritional adequacy
  • Educate residents, family members, and team members on nutrition and hydration recommendations
  • Participate in interdisciplinary teams and committees
Qualifications:
  • Registered Dietitian, eligible for membership with the College of Dietitians of Ontario
  • Extensive knowledge of geriatric nutrition
  • 3-5 years experience in long-term care setting preferred
  • Excellent organizational, time management, and multi-tasking skills
  • Knowledge of Occupational Health & Safety practices, principles, and legislation
  • Excellent oral and written communication skills
  • Customer-focused attitude, with demonstrated ability to build and maintain effective team relationships
