Southbridge Care Homes is seeking a skilled Registered Dietitian to join our Clinical Team.

As a member of our team, you will be responsible for providing expert nutrition advice and care to our residents.

Key Responsibilities:

  • Conduct comprehensive nutritional assessments for each resident using RAI MDS 2.0 as required.
  • Determine individualized resident diets and other nutritional interventions to meet their needs, involving residents, family members, and care teams.
  • Develop, implement, and evaluate nutritional care plans, identifying nutritional concerns, goals, and interventions for all residents.
  • Maintain excellent communication with the Dietary Manager regarding changing resident needs.
  • Review seasonal menus for nutritional adequacy and provide written approval.
  • Educate residents, family members, and team members on nutrition and hydration recommendations.
  • Participate in interdisciplinary teams, representing Southbridge Care Homes through regional and professional activities as needed.
  • Participate in committees related to resident care, facility operation, and quality indicators as required.

Qualifications:

  • Registered Dietitian, eligible for membership with the College of Dietitians of Ontario.
  • Extensive knowledge of geriatric nutrition.
  • 3-5 years of experience in long-term care settings preferred.
  • Excellent organizational, time management, and multi-tasking skills.
  • Knowledge of Occupational Health & Safety practices, principles, and legislation.
  • Excellent oral and written communication skills.
  • Customer-focused attitude with demonstrated ability to build and maintain effective team relationships with staff and residents.
