Empower Simcoe, a leading provider of community-based services in Simcoe County, seeks dedicated professionals to join our team as Specialized Resource Relief Coordinators.

About the RoleThe Specialized Resource Relief Coordinator will work as part of a support team to provide quality support to individuals with complex care needs residing in Treatment Homes and Specialized Resource locations.
- Utilize a personal outcome philosophy-based approach to support individuals in making informed choices that lead to successful attainment of their goals and aspirations;
- Provide guidance and direct support in areas identified as necessary related to activities of daily living, personal care, communication, emotional, physical, and mental health, safety, finances, relationship development to foster independence;
To be considered for this role, you must possess:
- A Community College Diploma in Social Services or a closely related field from an accredited college;
- Minimum of 1 year recent full-time work experience supporting hard-to-serve adults with complex care needs in community residential settings;
