Job Summary
Brightshores is seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities
- Assist with routine activities of patient care, working collaboratively with healthcare professionals to ensure optimal patient outcomes.
- Perform cross-functional duties, including administrative tasks and patient assessment, to support the delivery of high-quality patient care.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless communication and coordination of care.
- Participate in ongoing education and professional development to stay current with best practices and industry developments.
- Contribute to a safe and respectful work environment, adhering to Brightshores' 'CARE' Behaviours and 'LEAD' Competencies.
Requirements
- Certificate of Completion of the Personal Support Worker Program from a recognized Community College or equivalent experience and education.
- Current CPR certification.
- Recent experience working in a healthcare facility.
- Excellent written and verbal communication skills.
- Working knowledge of Infection Control Practices.
- Ability to work collaboratively within a multi-disciplinary team.
- Excellent organizational and patient assessment skills.
- Commitment to continuing education and professional development.
- Ability to work all shifts.
- Recent satisfactory performance and attendance record.
