Job Title: Support Worker

We are seeking a highly motivated and compassionate Support Worker to join our team at Chezranti Community Centre Ltd. As a Support Worker, you will play a vital role in providing support and guidance to individuals in need, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities:
  • Provide individualized support and guidance to clients, helping them to set and achieve their goals.
  • Assess clients' needs and develop personalized plans to address their specific challenges.
  • Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive support for clients.
  • Develop and implement life skills workshops and programs to help clients build confidence and independence.
  • Conduct regular assessments and evaluations to monitor clients' progress and adjust support plans as needed.
Requirements:
  • 2 years of experience in a related field, such as social work or counseling.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
  • Ability to work independently and as part of a team, with a high level of flexibility and adaptability.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
