Key Responsibilities- Provide wound and ostomy care to patients in the clinic and community
- Assess and manage wounds and ostomies using evidence-based practice
- Develop and implement individualized care plans to promote optimal patient outcomes
- Collaborate with interdisciplinary teams to achieve patient goals
- Provide education and support to patients and caregivers on wound and ostomy care
- Current College of Nurses of Ontario (CNO) registration in good standing
- Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET)
- Several years of escalating experience managing wounds and ostomies
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
