Your Opportunity:

At Alberta Health Services, we are seeking a skilled Licensed Practical Nurse to join our team as a Home Health Nurse Specialist. In this role, you will have the opportunity to work with clients in their own residences or alternate care settings, providing nursing assessments, case coordination, and direct care. You will be part of an interprofessional healthcare team, utilizing nursing processes, critical thinking, problem-solving, and decision-making skills to provide safe, quality patient and family-centered care.

Key Responsibilities:
  • Conduct comprehensive nursing assessments to identify client needs and develop care plans
  • Provide direct nursing care, including wound care, skin and ostomy care, and palliative support
  • Collaborate with clients and their families to ensure services are provided and modified on an ongoing basis
  • Work with the Home Care Manager to coordinate and provide quality care to clients in the community

Requirements:
  • Completion of an accredited practical nurse education program
  • Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Two years of recent home care or acute care experience in the last five years

Preferred Qualifications:
  • Experience working with acute clients and/or long-term clients with a focus on chronic disease management
  • Experience in providing case management services
  • Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support, and client teaching
