About the Role

CarePartners is seeking a compassionate and dedicated Home Support Worker to join our team. As a Home Support Worker, you will provide professional care and support to patients in their homes and residences in the community.

Key Responsibilities

  • Assist with activities of daily living, such as bathing, showering, personal hygiene, dressing, meal preparation, transferring/repositioning, light housekeeping, bowel/bladder care, and socializing and companionship.
  • Provide emotional support and companionship to patients and their families.
  • Work collaboratively with healthcare professionals to develop and implement care plans.
  • Maintain accurate records and reports as required.
