The Centretown Community Health Centre is seeking a skilled Community Health Worker to join our team in preventing chronic diseases among Ottawa's immigrant population. This regular part-time position involves working 21 hours per week, with a pay scale of $28.95 to $34.88/hourly.

As a Community Health Worker, you will be responsible for building relationships with community health and resource centres, chronic disease partners, and community leaders and volunteers. You will also be responsible for developing and implementing chronic disease screening event models as per the communities' unique requirements.

Responsibilities
