Job Summary:

We are seeking a compassionate Registered Practical Nurse (RPN) to join our team at Bayshore HealthCare. The ideal candidate will have excellent clinical skills, strong communication abilities, and a passion for delivering high-quality home healthcare.

About the Role:

This is a 16-week restorative homecare program in partnership with Kingston Health Sciences Centre (KHSC) and Bayshore HealthCare. As an RPN, you will provide nursing care to clients with varying diagnoses, abilities, and goals in their homes. You will use your clinical skills to ensure that our clients receive the highest quality home health care, delivered with compassion, excellence, and reliability.

Key Responsibilities:

  • Provide client care by utilizing observation, assessment, nursing diagnosis, counseling, and health teaching.
  • Develop and implement individualized care plans to meet the emotional, spiritual, physical comfort, and safety needs of clients.
  • Collaborate with interdisciplinary teams, including Physiotherapy, Occupational Therapy, Rehab Assistants, Speech Language Pathology, Social Work, Personal Support Work, and Dietician Services.
  • Document client conditions and changes according to proper standards and professional guidelines.
  • Participate in ongoing internal and external continuing education activities to maintain current knowledge and skills.
  • Adhere to Bayshore HealthCare policies and procedures, as well as provincial regulations and standards of practice.

Requirements:

  • Diploma or degree from an accredited Licensed/Registered Practical Nursing Program.
  • Current registration with the provincial licensing body.
  • Previous experience in community care, acute care sector, long-term care, or demonstrated clinical competencies.
  • Strong interpersonal and communication skills.
  • Critical thinking and analytical skills.
  • Ability to prioritize tasks and manage time effectively.
  • Valid G-level driver's license and reliable transportation.

What We Offer:

  • A competitive salary range of $90,000 - $110,000 per year, depending on experience.
