Nurse Specialist

Fraser Health is seeking a skilled Registered Nurse/Registered Psychiatric Nurse to join our team at the Hospice Residence. As a member of our multidisciplinary team, you will provide direct nursing care to patients, assessing their physical, emotional, spiritual, and psychosocial needs, and developing and implementing individualized care plans.
Responsibilities- Assess patients' needs and develop comprehensive care plans, collaborating with other healthcare professionals as needed.
- Provide high-quality nursing care, including administering medications, performing procedures, and monitoring patients' conditions.
- Communicate effectively with patients, families, and healthcare teams, ensuring seamless care coordination.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to evidence-based practice.
- Maintain accurate and timely documentation, adhering to regulatory requirements and Fraser Health's standards.
- Collaborate with other healthcare professionals to ensure comprehensive care, including physicians, therapists, and social workers.
- Stay up-to-date with the latest research, best practices, and guidelines in palliative care, applying this knowledge to improve patient outcomes.
- Contribute to a positive work environment, promoting a culture of respect, empathy, and teamwork.
Graduation from an approved School of Nursing and/or Psychiatric Nursing, with a minimum of two years' recent experience in medical nursing, including one year in a hospice/palliative care environment. Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
