About the Role

The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating care delivery and ensuring the highest quality of life for our residents. As a LPN, you will be the role model and team leader to an assigned unit, working closely with the Director of Care and other healthcare professionals to develop and implement individualized care plans.

Responsibilities
  • Assess residents' needs and develop comprehensive care plans that address their physical, emotional, and social well-being.
  • Liaise with physicians, community agencies, families, and other stakeholders to ensure seamless care coordination.
  • Administer medications and treatments as prescribed, and monitor residents' responses to care.
  • Participate in interdisciplinary care conferences and contribute to the development of care plans.
  • Support the administrative requirements of the community, including admissions and discharge processes.
  • Actively participate in orientation of new staff and community programs.
  • Provide coaching and direction to care staff as needed, and use in-the-moment education to improve resident care.
Qualifications and Skills
  • Graduate of an approved program for Practical Nursing and current licensure with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long-term care facility an asset.
  • Ability to read, write, and speak English proficiently and understand verbal and written instructions.
  • Computer skills, including the ability to navigate basic computer programs such as Word, Excel, and Outlook.
  • Demonstrates collaborative and team-working skills.
  • Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required to perform the duties of the position.
