Job SummaryWe're seeking a highly skilled Care Coordinator to join our team. As a key member of our Patient Services department, you'll play a vital role in assessing and determining patient care needs, providing access to community services, and coordinating care plans.
Responsibilities- Assess patient care needs and determine eligibility for services
- Link patients with service providers and coordinate care plans
- Develop individual care and service plans in collaboration with patients and their families
- Balance patient needs and choices with available resources
- Membership in good standing with applicable regulatory body
- University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, or Social Work
- 2+ years of recent experience in community health or a related field
- Knowledge of healthcare delivery system and community resources
- Excellent interpersonal and communication skills
