About the Role

We are seeking an experienced Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to join our team as a Forensic Community Healthcare Specialist. This is a unique opportunity to work with individuals with complex mental health and substance use challenges in a community setting.

Key Responsibilities

  1. Conduct comprehensive biopsychosocial assessments, including initial intake and ongoing evaluations, to identify client needs and develop Integrated Treatment Plans.
  2. Provide community case coordination and advocacy services, ensuring continuity of care and seamless communication with mental health teams, specialists, and social service agencies.
  3. Maintain accurate and concise documentation of client files, adhering to FPSC policy standards and regulatory requirements.
  4. Offer nursing services, information, and support, including short-term follow-up, counseling, and therapeutic support to individual clients and their families.
  5. Facilitate access to psychiatric and social services, receiving referrals, accompanying clients to community resources, and conducting home visits as per the ITP and FPSC policy.

Requirements

  • Diploma in Nursing or Psychiatric Nursing.
  • Two years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
  • Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
  • Valid BC Drivers License.
